Elle se caract\u00e9rise aussi par une faible capacit\u00e9 cardio-pulmonaire. Nous avons test\u00e9 l\u2019effet de l\u2019exercice physique et d\u2019une suppl\u00e9mentation en pr\u00e9curseur du NAD\u207a par <strong>nicotinamide riboside<\/strong>, deux interventions ayant chacune montr\u00e9 des b\u00e9n\u00e9fices dans des \u00e9tudes animales et dans des \u00e9tudes cliniques pr\u00e9coces, sur la capacit\u00e9 cardio-pulmonaire de personnes atteintes d\u2019ataxie de Friedreich.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>M\u00e9thodes<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Il s\u2019agissait d\u2019un essai clinique de phase 2<sup>[1]<\/sup>, ambulatoire<sup>[2]<\/sup>, monocentrique<sup>[3]<\/sup>, randomis\u00e9<sup>[4]<\/sup>, avec un plan factoriel 2 \u00d7 2<sup>[5]<\/sup>, d\u2019une dur\u00e9e de 12 semaines. L\u2019\u00e9tude a \u00e9t\u00e9 men\u00e9e au <strong>Children\u2019s Hospital of Philadelphia<\/strong>, \u00e0 Philadelphie, aux \u00c9tats-Unis. Elle a recrut\u00e9 des personnes \u00e2g\u00e9es de 10 \u00e0 40 ans, ayant une fraction d\u2019\u00e9jection d\u2019au moins 45 % et capables de faire de l\u2019exercice.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Une s\u00e9quence de randomisation g\u00e9n\u00e9r\u00e9e par ordinateur a \u00e9t\u00e9 \u00e9labor\u00e9e par le statisticien de l\u2019essai. La r\u00e9partition al\u00e9atoire \u00e9tait stratifi\u00e9e selon l\u2019\u00e2ge \u2014 moins de 18 ans versus 18 ans ou plus \u2014 dans l\u2019un des quatre groupes suivants :<\/p>\n\n\n\n<ol start=\"1\" class=\"wp-block-list\">\n<li>placebo sans exercice, avec contr\u00f4le attentionnel par appels t\u00e9l\u00e9phoniques hebdomadaires \u2014 appel\u00e9 ensuite groupe placebo seul ;<\/li>\n\n\n\n<li>nicotinamide riboside sans exercice, avec contr\u00f4le attentionnel \u2014 appel\u00e9 ensuite groupe nicotinamide riboside seul ;<\/li>\n\n\n\n<li>placebo avec exercice \u2014 appel\u00e9 ensuite groupe exercice seul ;<\/li>\n\n\n\n<li>nicotinamide riboside avec exercice \u2014 appel\u00e9 ensuite traitement combin\u00e9.<\/li>\n<\/ol>\n\n\n\n<p class=\"wp-block-paragraph\">Des programmes d\u2019exercice individualis\u00e9s ont \u00e9t\u00e9 \u00e9labor\u00e9s par le physiologiste de l\u2019exercice. Ils comprenaient trois s\u00e9ances d\u2019entra\u00eenement a\u00e9robie et deux s\u00e9ances de renforcement musculaire par semaine. Les s\u00e9ances \u00e9taient r\u00e9alis\u00e9es au domicile des participants et supervis\u00e9es \u00e0 distance, notamment par des appels t\u00e9l\u00e9phoniques du physiologiste.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">La dose de nicotinamide riboside ou de placebo \u00e9tait adapt\u00e9e au poids :<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>300 mg, soit 1 capsule, pour un poids de 24 kg \u00e0 moins de 48 kg ;<\/li>\n\n\n\n<li>600 mg, soit 2 capsules, pour un poids de 48 kg \u00e0 moins de 72 kg ;<\/li>\n\n\n\n<li>900 mg, soit 3 capsules, pour un poids sup\u00e9rieur \u00e0 72 kg.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Le crit\u00e8re principal \u00e9tait la variation du <strong>pic de VO\u2082<strong><sup>[6]<\/sup><\/strong><\/strong>, exprim\u00e9 en L\/min, mesur\u00e9 lors d\u2019un test d\u2019effort cardio-pulmonaire apr\u00e8s 12 semaines par rapport \u00e0 la valeur initiale. L\u2019effet du groupe de traitement a \u00e9t\u00e9 \u00e9valu\u00e9 \u00e0 l\u2019aide d\u2019un mod\u00e8le statistique tenant compte de l\u2019\u00e2ge, qui \u00e9tait la variable de stratification, du sexe et du pic de VO\u2082 initial.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">L\u2019analyse de premi\u00e8re \u00e9tape comparait chaque traitement actif au groupe contr\u00f4le. L\u2019analyse de deuxi\u00e8me \u00e9tape, si le traitement combin\u00e9 \u00e9tait efficace, comparait le traitement combin\u00e9 \u00e0 l\u2019exercice seul. Le risque global d\u2019erreur de type 1 <sup>[7]<\/sup>\u00e9tait maintenu \u00e0 moins de 0,05. Les analyses ont \u00e9t\u00e9 r\u00e9alis\u00e9es en intention de traiter<sup>[8]<\/sup>. Les \u00e9v\u00e9nements ind\u00e9sirables ont \u00e9t\u00e9 recueillis de mani\u00e8re syst\u00e9matique. L\u2019essai est enregistr\u00e9 sur ClinicalTrials.gov sous le num\u00e9ro <strong>NCT04192136<\/strong> et il est termin\u00e9.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>R\u00e9sultats<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Entre le 3 septembre 2020 et le 23 avril 2025, 74 personnes ont \u00e9t\u00e9 incluses, dont 66 r\u00e9pondaient aux crit\u00e8res d\u2019\u00e9ligibilit\u00e9 et ont \u00e9t\u00e9 r\u00e9parties al\u00e9atoirement dans les quatre groupes de l\u2019\u00e9tude. Tous les participants ont termin\u00e9 l\u2019\u00e9tude.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Parmi eux, 33 participants, soit 50 %, \u00e9taient des enfants \u00e2g\u00e9s de 10 \u00e0 17 ans, et 33, soit 50 %, \u00e9taient des adultes \u00e2g\u00e9s de 18 ans ou plus. Trente-sept participants, soit 56 %, \u00e9taient de sexe masculin, et 29, soit 44 %, de sexe f\u00e9minin.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Les moyennes des moindres carr\u00e9s<sup>[9]<\/sup> pour la variation du pic de VO\u2082, en L\/min, \u00e9taient les suivantes :<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>-0,05<\/strong> ; intervalle de confiance \u00e0 95 % de -0,16 \u00e0 0,06, chez les 17 participants du groupe contr\u00f4le ;<\/li>\n\n\n\n<li><strong>0,06<\/strong> ; intervalle de confiance \u00e0 95 % de -0,05 \u00e0 0,17, chez les 17 participants du groupe nicotinamide riboside sans exercice ;<\/li>\n\n\n\n<li><strong>0,11<\/strong> ; intervalle de confiance \u00e0 95 % de 0,00 \u00e0 0,22, chez les 16 participants du groupe placebo avec exercice ;<\/li>\n\n\n\n<li><strong>0,16<\/strong> ; intervalle de confiance \u00e0 95 % de 0,05 \u00e0 0,27, chez les 16 participants du groupe nicotinamide riboside avec exercice.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Les diff\u00e9rences entre les traitements actifs et le groupe contr\u00f4le \u00e9taient :<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>0,10<\/strong> ; intervalle de confiance \u00e0 95 % de -0,05 \u00e0 0,26 ; <em>p<\/em> ajust\u00e9 = 0,188, pour le nicotinamide riboside sans exercice ;<\/li>\n\n\n\n<li><strong>0,16<\/strong> ; intervalle de confiance \u00e0 95 % de 0,00 \u00e0 0,31 ; <em>p<\/em> ajust\u00e9 = 0,103, pour le placebo avec exercice ;<\/li>\n\n\n\n<li><strong>0,21<\/strong> ; intervalle de confiance \u00e0 95 % de 0,05 \u00e0 0,36 <strong>; <em>p<\/em> ajust\u00e9 = 0,0299,<\/strong> pour l\u2019association nicotinamide riboside et exercice<sup>[10]<\/sup>.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Le traitement combin\u00e9 n\u2019\u00e9tait pas statistiquement diff\u00e9rent de l\u2019exercice seul : diff\u00e9rence de -0,05 ; intervalle de confiance \u00e0 95 % de -0,10 \u00e0 0,21 ; p = 0,49.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Tous les \u00e9v\u00e9nements ind\u00e9sirables \u00e9taient l\u00e9gers ou mod\u00e9r\u00e9s. Ils comprenaient des sympt\u00f4mes gastro-intestinaux, des chutes, des infections des voies respiratoires sup\u00e9rieures et des \u00e9ruptions cutan\u00e9es.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Au moins un \u00e9v\u00e9nement ind\u00e9sirable mod\u00e9r\u00e9 d\u2019int\u00e9r\u00eat dans ces cat\u00e9gories a \u00e9t\u00e9 rapport\u00e9 chez :<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>7 participants, soit 41 %, dans le groupe contr\u00f4le ;<\/li>\n\n\n\n<li>6 participants, soit 35 %, dans le groupe nicotinamide riboside sans exercice ;<\/li>\n\n\n\n<li>3 participants, soit 19 %, dans le groupe placebo avec exercice ;<\/li>\n\n\n\n<li>4 participants, soit 25 %, dans le groupe nicotinamide riboside avec exercice.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Interpr\u00e9tation<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">L\u2019association de <strong>nicotinamide riboside et d\u2019exercice physique pendant 12 semaines<\/strong> \u00e9tait s\u00fbre et a augment\u00e9 la capacit\u00e9 cardio-pulmonaire chez des enfants et des adultes atteints d\u2019ataxie de Friedreich.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Des \u00e9tudes plus longues sont n\u00e9cessaires pour \u00e9tablir si l\u2019ajout du nicotinamide riboside \u00e0 l\u2019exercice physique pourrait \u00eatre envisag\u00e9 comme \u00e9l\u00e9ment d\u2019une strat\u00e9gie th\u00e9rapeutique globale \u00e0 long terme.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong><u>Figure 2. Les augmentations sont indiqu\u00e9es en rouge, et les diminutions en bleu. Les barres grises montrent les valeurs initiales \u00e0 la semaine 0.<\/em><\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><em>Les cercles repr\u00e9sentent les adultes, \u00e2g\u00e9s de 18 ans ou plus, et les triangles repr\u00e9sentent les enfants, \u00e2g\u00e9s de moins de 18 ans. Ils indiquent les valeurs \u00e0 la semaine 12 ; l\u00e0 encore, les valeurs ayant augment\u00e9 par rapport au niveau initial sur les 12 semaines sont en rouge, et celles ayant diminu\u00e9 sont en bleu.<\/em><\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><em>Le nombre de personnes incluses dans chaque analyse est indiqu\u00e9 par <strong>N<\/strong>. Dans le groupe placebo seul, une personne n\u2019a pas r\u00e9alis\u00e9 le test d\u2019effort cardio-pulmonaire \u00e0 la semaine 12 en raison d\u2019une blessure au pied ; elle n\u2019a donc pas \u00e9t\u00e9 incluse dans les r\u00e9sultats portant sur ce test.<\/em><\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<p class=\"has-small-font-size wp-block-paragraph\">[1] Un <strong>essai clinique de phase 2<\/strong> est une \u00e9tude r\u00e9alis\u00e9e chez des patients pour savoir si un traitement est <strong>prometteur<\/strong> et <strong>suffisamment s\u00fbr<\/strong> pour \u00eatre \u00e9tudi\u00e9 plus largement.<\/p>\n\n\n\n<p class=\"has-small-font-size wp-block-paragraph\">[2] <strong>Ambulatoire<\/strong> : les participants ne sont pas hospitalis\u00e9s. Ils viennent pour les \u00e9valuations, mais vivent chez eux pendant l\u2019\u00e9tude.<\/p>\n\n\n\n<p class=\"has-small-font-size wp-block-paragraph\">[3] <strong>Monocentrique<\/strong> : l\u2019\u00e9tude est r\u00e9alis\u00e9e dans un seul centre m\u00e9dical, ici le Children\u2019s Hospital of Philadelphia.<\/p>\n\n\n\n<p class=\"has-small-font-size wp-block-paragraph\">[4] <strong>Randomis\u00e9<\/strong> : les participants sont r\u00e9partis au hasard dans les diff\u00e9rents groupes, pour \u00e9viter de fausser les r\u00e9sultats.<\/p>\n\n\n\n<p class=\"has-small-font-size wp-block-paragraph\">[5] <strong>Plan factoriel 2 \u00d7 2<\/strong> : l\u2019\u00e9tude teste deux interventions en m\u00eame temps :<\/p>\n\n\n\n<ol class=\"wp-block-list\">\n<li class=\"has-small-font-size\">le <strong>nicotinamide riboside<\/strong> ou son placebo ;<\/li>\n\n\n\n<li class=\"has-small-font-size\">l\u2019<strong>exercice physique<\/strong> ou l\u2019absence d\u2019exercice.<\/li>\n<\/ol>\n\n\n\n<p class=\"has-small-font-size wp-block-paragraph\">Cela donne quatre groupes. Cette \u00e9tude permet de comparer s\u00e9par\u00e9ment l\u2019effet du nicotinamide riboside, l\u2019effet de l\u2019exercice, et l\u2019effet de leur association.<\/p>\n\n\n\n<p class=\"has-small-font-size wp-block-paragraph\">[6] Le <strong>pic de VO\u2082<\/strong> est mesur\u00e9 lors d\u2019un test d\u2019effort cardio-pulmonaire. Il indique la quantit\u00e9 maximale d\u2019oxyg\u00e8ne que la personne peut utiliser quand elle fait un exercice intense. La <strong>variation du pic de VO\u2082<\/strong> correspond au changement de la capacit\u00e9 maximale du corps \u00e0 utiliser l\u2019oxyg\u00e8ne pendant un effort.<\/p>\n\n\n\n<p class=\"has-small-font-size wp-block-paragraph\">[7] Le <strong>risque d\u2019erreur de type 1<\/strong> correspond au risque de conclure \u00e0 tort qu\u2019un traitement est efficace alors qu\u2019en r\u00e9alit\u00e9 il ne l\u2019est pas. C\u2019est comme dire : \u00ab il y a un effet du traitement \u00bb, alors que cet effet est d\u00fb au hasard. Le <strong>risque global d\u2019erreur de type 1<\/strong> devient important quand une \u00e9tude fait <strong>plusieurs comparaisons statistiques<\/strong>. Plus on multiplie les tests, plus on augmente le risque de trouver par hasard un r\u00e9sultat \u201csignificatif\u201d.<\/p>\n\n\n\n<p class=\"has-small-font-size wp-block-paragraph\">[8] Les analyses r\u00e9alis\u00e9es <strong>en intention de traiter<\/strong> signifient que chaque participant est analys\u00e9 dans le groupe o\u00f9 il a \u00e9t\u00e9 tir\u00e9 au sort au d\u00e9part, m\u00eame s\u2019il n\u2019a pas parfaitement suivi le traitement ou le programme pr\u00e9vu.<\/p>\n\n\n\n<p class=\"has-small-font-size wp-block-paragraph\">[9] La <strong>moyenne des moindres carr\u00e9s<\/strong> est une moyenne <strong>ajust\u00e9e statistiquement<\/strong>.<\/p>\n\n\n\n<p class=\"has-small-font-size wp-block-paragraph\">Elle ne correspond pas simplement \u00e0 la moyenne brute observ\u00e9e dans un groupe. Elle tient compte de certains facteurs qui peuvent influencer le r\u00e9sultat ( par exemple ici&nbsp; l\u2019\u00e2ge, le sexe, et la valeur du pic de VO2 initial)<\/p>\n\n\n\n<p class=\"has-small-font-size wp-block-paragraph\">[10] Ce r\u00e9sultat est consid\u00e9r\u00e9 comme statistiquement significatif, pour le traitement combin\u00e9 versus placebo, car\u00a0: <strong>Association nicotinamide riboside + exercice :<\/strong> diff\u00e9rence = <strong>0,21<\/strong> ; intervalle de confiance \u00e0 95 % <strong>0,05 \u00e0 0,36<\/strong> ; <strong>p ajust\u00e9 = 0,0299<\/strong>.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li class=\"has-small-font-size\">la valeur de <strong>p ajust\u00e9 est inf\u00e9rieure \u00e0 0,05<\/strong> ;<\/li>\n\n\n\n<li class=\"has-small-font-size\">l\u2019intervalle de confiance est enti\u00e8rement au-dessus de 0 : il va de <strong>0,05 \u00e0 0,36<\/strong> ;<\/li>\n\n\n\n<li class=\"has-small-font-size\">cela signifie que l\u2019am\u00e9lioration observ\u00e9e du pic de VO\u2082 est peu compatible avec le simple hasard statistique.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\"><\/p>\n","protected":false},"excerpt":{"rendered":"<p>Lin KY, Bucha A, McSweeney K, Wade KL, Karaj A, Tamaroff J, O&#8217;Malley S, Chung NM, Cilenti NA, Wanner J, Adzika GK, Mesaros C, Blair IA, Rojsajjakul T, Serai S, Farmer J, Bryant K, Lu Y, Harhay MO, Weber DR, Paridon SM, Seifert EL, Putt ME, Zamani P, Baur JA, Lynch DR, McCormack SE.
